Nausea before or during your period: why it happens and what helps
Where cyclical nausea comes from, how to tell it apart from pregnancy, and what can actually bring relief.

Feeling nauseous for no obvious reason, right before your period, is confusing and exhausting. It is not in your head. Nearly 3 in 4 women report at least one digestive symptom before or during their period, and nausea is one of the most common [1]. This nausea has a physical cause, and real solutions.
Prostaglandins and hormones: the physical cause of your nausea
Your nausea has a physical cause, not a psychological one. Just before and during your period, the uterine lining releases prostaglandins (molecules that make the uterus contract to shed blood). They enter the bloodstream and irritate the stomach and intestines [2]. The result: nausea, cramps, sometimes diarrhea.
In women with painful periods (dysmenorrhea), these prostaglandins run higher [3]. It's the same mechanism behind both period pain and nausea. Nearly 9 in 10 women with dysmenorrhea report nausea or vomiting [3].
Your hormones matter too. Progesterone slows digestion. Its sharp drop before your period unsettles the stomach, and its by-products act on the brain, which can trigger the nausea of premenstrual syndrome (PMS) [4]. PMS affects nearly 1 in 2 women of reproductive age [5].
Some women also feel this nausea mid-cycle, at ovulation. As the follicle releases the egg, it spills fluid rich in prostaglandins that irritates the belly: this is Mittelschmerz (a brief pain on one side of the lower abdomen) [4].

How to tell period nausea from pregnancy nausea
This is often the worry that lingers: "what if I'm pregnant?" Here's how to see clearly.
Cycle-related nausea is predictable. It returns at the same point each month, lasts 1 to 3 days, then fades, and your period arrives as usual.
Pregnancy nausea comes from the hormone hCG, produced after the embryo implants [6]. It is daily, often worse in the morning, comes with a missed period, and lasts for weeks. It affects nearly 3 in 4 pregnant women in the first trimester [6].
When in doubt, a simple pregnancy test settles it. There is no shame in taking one for peace of mind, even if you're using contraception.

Ginger, smaller meals, NSAIDs: what can help
You are not doomed to endure this nausea. Several options exist, to combine based on how intense your symptoms are:
- Ginger, 1 to 1.5 g a day (as tea, capsules, or freshly grated), acts on the nausea receptors in the stomach and brain [7].
- Small, frequent meals (5 to 6 light ones), avoiding fatty, spicy, and very sugary foods. Sip water in small amounts, and peppermint can also soothe the stomach.
- Anti-inflammatories (ibuprofen, mefenamic acid) if your periods are painful: they lower prostaglandins [3]. You can start them 1 to 2 days before your period.
For severe PMS, combined hormonal contraceptives can steady your hormones and reduce symptoms [5]. For intense, stubborn nausea, your doctor can prescribe a short course of an antiemetic. It's fair to ask for treatment that matches what you're going through.

When your nausea may point to endometriosis
In most cases, this nausea is uncomfortable but manageable. Sometimes, though, your body is signaling something else.
With endometriosis, tissue similar to the uterine lining grows outside the uterus, sometimes on the intestines or bladder [8]. It responds to your cycle's hormones and produces prostaglandins locally. When it reaches the digestive tract, it can cause intense nausea, bloating, and bowel changes that return with every cycle [8]. These symptoms are so close to irritable bowel syndrome that they are often mistaken for it [8].
Endometriosis affects about 1 in 10 women of reproductive age, some 1.5 to 2.5 million women in France [8]. The wait for a diagnosis still averages 7 years: this is neither acceptable nor your fault. If your nausea follows your cycle and keeps worsening, you have every right to ask for a thorough evaluation and a second opinion. Your experience is valuable information.

Your cyclical nausea has a clear physical cause and, above all, solutions. You can try ginger and smaller, more frequent meals as early as your next cycle. If it's intense, worsening, or resistant, speak up without fear of being dismissed. Logging your nausea alongside your cycle in Endora helps you spot a pattern to show your doctor.
